O śluzie szyjkowym raz jeszcze

Bardzo szczegółowymi badaniami nad śluzem szyjkowym zajmuje się już od lat 60tych poprzedniego wieku szwedzki uczony Eric Odeblad. O tym, że rozróżniamy śluz gestagenny typu G oraz śluzy typu E (E-L i E-S), czyli estrogenozależne, pisałam kilka notek wcześniej.

Ta różnorodność śluzu w różnych fazach cyklu wynika z aktywności poszczególnych krypt w szyjce macicy. Odeblad zbadał i opisał szczegółowy skład chemiczny tych śluzów, oraz wyróżnił jeszcze śluz typu P i F. Hormony jajnikowe uwalniane w organizmie kobiety cyklicznie powodują, że proporcje śluzu typu L, S, P i G również się zmieniają. Z tych wymienionych typów, największe znaczenie ma śluz typu L, S i G. Ten ostatni, będąc gęstym i lepkim zamyka ujście szyjki macicy czyniąc ją nieprzepuszczalną dla plemników. A śluz estrogenozależny, jest mieszaniną dwóch rodzajów śluzu: bochenkowatego, gęstego L (loaf – bochenek), między którym przepływa wodnisty, rzadki śluz S (string – sznurek).

Śluz typu L zwany jest śluzem typu mniej płodnego (mętny białawy, lepki, daje odczucie wilgotno), będzie krystalizował w kształcie liści paproci. Gęstość ma pośrednią między śluzami S i G, wydzielany jest jako drobne (1mm) grudki wypełniające kanał szyjki macicy przy średnich i rosnących poziomach estrogenów. Odpowiada za sekrecję plemników.

Śluz typu S, czyli ten wysoce płodny (ciągnący się, płynny, wodnisty), w obrazie mikroskopowym będzie krystalizował jako równoległe igły, krypty szyjki macicy wytwarzają go po pobudzeniu przez wysokie stężenie estrogenów. Ten rodzaj śluzu bez problemu przepływa między grudkami śluzu L, a oprócz tego daje możliwość plemnikom na swobodne poruszanie się. Co ciekawe, plemniki są w górnych kryptach szyjki magazynowane i w kierunku jajowodów wysyłane sukcesywnie.

Śluz typu P (peak –szczyt) pojawia się w kryptach szyjki macicy ok. 36 godzin przed szczytem estrogenów i działa krótko. Najważniejsze z jego podtypów to P6 i Pa. Ten pierwszy powoduje uruchomienie plemników znajdujących się w kryptach, a ten drugi pomaga w ich przeprowadzce w kierunku jajników. Aby śluz P miał swoje właściwości, konieczna jest jeszcze wydzielina Z, która zawiera enzymy działające na plemniki.

Śluz F (fundamental – podstawowy) wytwarzany jest przez nabłonek między kryptami szyjki macicy i obecny jest przez cały czas trwania cyklu.

Jak się okazuje, krypty typu S (będące w przewadze liczebnej w okresie dojrzewania kobiety) wraz ze starzeniem się szyjki macicy, zastępowane są kryptami L. Istnieje jeden sposób, by szyjkę macicy nieco odmłodzić - ciąża stymuluje powstawanie krypt typu S i odmładza szyjkę o 2-3 lata. :) Niestety można również proces jej starzenia przyśpieszyć. Na przykład antykoncepcja hormonalna powoduje dwukrotnie szybsze jej starzenie.

Na cykliczność śluzowych zmian, oprócz estrogenów i progesteronu ma wpływ również mangan, który wydzielany jest w pochwie przez kieszonkę Showa(pobudzaną hormonalnie). Gdy jest ona nieaktywna i nie wydziela manganu, śluz może swobodnie spływać do przedsionka pochwy dając uczucie mokrości/śliskości. Natomiast uwalniający się z kieszonki mangan działa odwadniająco na śluz, powoduje jego gęstnienie i odczucie suchości.

Obrazek przedstawia schematyczne rozmieszczenie krypt w szyjce macicy. Śluz typu L zajmuje największy obszar błony śluzowej, krypty śluzu typu S znajdują się w górnej części szyjki a śluz typu G wytwarzany jest w części szyjki dolnej.
